Pulse Holistic Health – Policies

Fees & Cancellations- All payments, therapy and herbs are due at the time of service. Any outstanding invoices will be billed one time and then forwarded to collections. Every month there is an outstanding invoice, a 14% interest fee will be applied to the balance until the balance is paid. A $25 fee will be applied to returned checks.

Appointments- At Pulse we understand that unanticipated events happen occasionally in everyone’s life and we are happy to honor your business as follows. We require 48-hour advance notice when cancelling therapy appointments at the discretion of the practitioner.

No Shows– Anyone who fails to show up for a scheduled appointment and does not provide 48 hours cancellation notice, will be considered a “no show” and will be charged the full amount for the missed appointment at the discretion of the practitioner. After 2 “no shows”, practitioners may require prepayment or a valid credit card to hold your next appointment

Late Arrivals- Out of respect and consideration to your therapist and other clients, please plan accordingly and be on time. If you arrive late, your session may be shortened in order to accommodate other whose appointments follow yours. Depending upon how late you arrive, your therapist will determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the full fee.

Client Files– The practitioners of Pulse Holistic Health are independent practitioners and as such, maintain their own clientele and files. Clients that seek treatment with one or more practitioner at Pulse Holistic Health may be asked to fill out independent paperwork, or grant permission for the practitioner to have access to their files from their primary or initial practitioner. Practitioners that leave Pulse Holistic Health take their files with them.

Herb Policy- Pulse Holistic Health sells herbs to clients of Pulse Holistic Health practitioners only. This excludes items that do not require assessment i.e., lung and throat drops or topicals. We will gladly accept returns on any unopened herbs for in office credit, we cannot accept returns on opened or expired herbs. Herb payment is due at the time of purchase.

Notice of Privacy Practices Uses and disclosure of health information- Pulse uses your health information for the purpose of treatment, obtaining payment, administrative uses and notification about specials, greetings and education purposes. Continuity use or disclose individually identifiable health information (IIHI) without your authorization for several reasons including auditing, research studies, emergency situations or when required by law. We reserve the right to change our policies at any time. We will post notices in the customer waiting area of any significant changes to be made.

Individual Rights-In most cases you have the right to look at or get a copy of your health information. Fees for photocopying and mailing may apply. You also have the right to receive a list of instances where your health information was disclosed for reasons other than treatment, payment or other administrative purposes.

Complaints- If you are concerned that your privacy rights have been violated or you disagree with a decision made about access to your records, please contact the privacy officer listed below. You may also send a written complaint to the US Department of Health and Human Services.

Our Legal Duty- We are required by law to protect the privacy of your information, provide this notice about our information practices, follow the information practices described in the notice and obtain your acknowledgment of receipt of notice.
